•A details about the setting page, including a space to record useful telephone numbers as well as parents/carers/emergency contact numbers.
•6 Medication permission forms. These are for you to gain permission from the parents/carers for giving Calpol, nappy cream etc (although you do still need to show that you have made every reasonable attempt to ask permission each time you give medication as in EYFS 2017)
•12 short term medication forms (3 medication records per page) These provide space for you to record whether the parent/carer has already given a dose of medication before bringing the child, along with the name of medication given, dose and time given. A space for the parent/carer to sign the medicine in with you, stating, when they would like you to give the medicine and how much to give. There is also 3 spaces for you to record any doses that you give along with any notes, and space for signatures from the childminder and parent/carer.
•6 long term medication forms for you to record any continuous medication such as inhalers etc..
